Our mission is to provide professional and paraprofessional services that are geared to- ward the recovery of adults living with a di-
agnosis of serious mental illness and having a record of an extended stay in psychiatric hospital, a record of frequent arrests, or a re-
cord of frequent emergency room visits. We continue to provide services that enable our clients achieve the highest level of potentials to successfully live in their chosen community.
We are committed to providing a high qual- ity, multi-disciplinary Home and Community Based Services, Adult Mental Health (HCBS- AMH) program through our staff
and subcon- tractors by tailoring individualized plan of care to clients’ unique journey to recovery. We en- courage our clients to be the driver of their own recovery processes.
We provide services in Dallas, Tarrant and Travis Counties
Specialized equipment and supplies including devices, controls, and appliances that enable individuals to more fully integrate into their community and increase their abilities to perform activities of daily living. These include calendars, timer, stretchers, wheel chairs and other items necessary for life support.
Personal assistance with activities of daily living such as grooming, eating, bathing, dressing, personal hygiene and functional living tasks. We assist with meal planning and preparation, transportation, mental health therapies/activities and assistance with medications in accordance with the State rules and guidelines.
24-hour on-site services in community-based home environment, with experienced staff in personal care, chores, medication oversight, therapeutic, social, and recreational activities. We can meet scheduled and unpredictable needs in a way that promotes maximum dignity, independence with supervision, safety and security.
Residential assistance to foster recovery and independence by helping with activities of daily living. These services also include assisting with other functional living tasks such as meal planning and preparation, transportation or assistance, ambulation and mobility, reinforcement of specialized rehabilitative, habilitative and psychosocial therapies.
Direct personal assistance in client, or client-family homes, to support activities of daily living. These also include assisting with other functional living tasks such as meal planning and preparation, transportation or assistance in securing transportation, ambulation and mobility, reinforcement of specialized rehabilitative, habilitative and psychosocial therapies.
Goal directed supports and solution-focused interventions, intended to achieve identified goals or objectives as defined in the individual’s IRP (Individualized Recovery Plan).
Support for individuals as they work at jobs of their choosing and to achieve goals meaningful to them, such as increasing their economic security.
Our dedicated sub-contractor help individual locate and maintain paid employment in the community and may include activities on behalf of the individual to assist in maintaining employment.
Christabel LLC, through our subcontractor provide services to sustain individuals in paid jobs in regular work settings, who, because of disability, require support to be self-employed, work from home, or perform in a work setting at which individuals without disabilities are employed.
We provide individual with temporary flexible funds with approval from DSHS after review for non-clinical supports that augment the IRP to reduce symptomatology and maintain quality of life and cornmupity integration. Peer Support- These services are provided through our subcontractors, who are
Provide nutritionally sound meals to individuals in their homes.
Physical adaptations to an individual’s home that are necessary to ensure individual’s health, welfare, and safety, or that enable the individual to function with greater independence in the home.
Nursing services are provided through our Registered Nurse or licensed Vocational nurse under the supervision of a Clinical Supervisor RN. Services include ongoing chronic conditions, wound care, medical administration and supervising delegated tasks.
Evidenced-based or evidence-informed interventions which support the individual’s recovery by helping the individual develop, refine and/or maintain the skills needed to function successfully in the community.
Temporary, planned or emergency short-term relief to the primary caregiver of an individual.
Provide assessment and ambulatory group and individual counseling for individuals experiencing substance use disorder. These services are specialized to meet the needs of individuals who have experienced extended institutional placement.
Payment of set-up expenses for individuals transitioning from institutions into community settings that is necessary to enable individuals to establish basic household needs.
Non-medical transportation provided that enables individuals to support their recovery goals and gain access to services, activities, and resources, as specified in the Individualized Recovery Plan (IRP).
